The t(15;17)(q22;q21) translocation is commonly associated with Acute Promyelocytic Leukemia (APL), a subtype of Acute Myeloid Leukemia (AML) with M3 morphology. APL accounts for approximately 10-15% of AML cases. This translocation results from the fusion of the PML gene (located on chromosome 15q22) and the RARa gene (located on chromosome 17q21). The breakpoint in the RARa gene is located within a 15 kb region of intron 2, while the breakpoint in the PML gene can occur in three different regions: intron 6 (55% of cases), intron 3 (40% of cases), and exon 6 (5% of cases). Each breakpoint is associated with a specific PML-RARa isoform named bcr1, bcr3, and bcr2, respectively. The fusion protein acts as a transcriptional repressor, blocking differentiation of hematopoietic stem cells and arresting them at the promyelocytic stage. Treatment with ATRA (All-Trans Retinoic Acid) is highly effective in APL, acting as an inducer of differentiation.
